By joining the SMS program provided by Horizon Recuperative Care, you agree to receive recurring text messages — notifications, updates, and reminders — at the number you provide, under these Terms, which adhere to the Telephone Consumer Protection Act (TCPA, 2026) and CTIA Best Practices.
1Opt-In Consent
In accordance with 2026 TCPA guidelines, participation in our SMS service requires explicit one-to-one consent. By opting in, you consent to receive messages from Horizon Recuperative Care only. Consent cannot be shared with other companies or third-party entities without your direct approval.
You may opt in through the following methods:
Provide consent by checking a clearly labeled checkbox explicitly stating SMS program enrollment on our contact form.
Complete and sign a paper intake form that includes a dedicated section outlining SMS consent.
2Message Frequency
Message frequency varies and will not exceed 3–5 messages per day unless there is a notification event. Standard carrier rates apply.
3Types of Messages
Messages from Horizon Recuperative Care may include appointment reminders, scheduling updates, service notifications, and care coordination messages.
4Opt-Out Process
Reply STOP at any time to end messages immediately.

We maintain detailed logs of opt-ins and opt-outs — dates, times, and methods — to satisfy TCPA proof-of-consent requirements.
